HIPAA Notice of Privacy Practices for Protected Health Information

 

Pharmacy To Me is committed to maintaining the privacy of your protected health information (PHI) and to providing you with quality care. Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we have a legal duty to protect the privacy of your health information. This Notice explains how we may use and disclose your health information and your rights concerning your health information.

Pharmacy To Me is required by law to:

  • Provide you with this Notice of our legal duties and privacy practices with respect to PHI.
  • Notify affected individuals following a breach of unsecured PHI.

How We May Use and Disclose Your PHI

 

For Treatment: We may use or disclose your PHI to provide, coordinate, or manage your health care and any related services. This includes the coordination or management of your health care with a third party for treatment purposes.

 

For Payment: Your PHI will be used, as needed, to obtain payment for the health care services we provide. This may include certain activities that your health insurance plan may undertake before it approves or pays for the health care services we recommend for you.

 

For Health Care Operations: We may use or disclose your PHI, as necessary, for our own health care operations to facilitate the function of Pharmacy To Me and to provide quality care to all of our customers. Health care operations include activities such as quality assessment and improvement, licensing, and conducting or arranging for other business activities.

 

Other Uses and Disclosures: As permitted or required by law, we may also use or disclose your PHI for law enforcement purposes, to avert a serious threat to health or safety, and in other situations as allowed by HIPAA.

 

Your Rights

 

You have the following rights regarding the PHI that we maintain about you:

  • Right to Inspect and Copy: You have the right to inspect and copy PHI that may be used to make decisions about your care.
  • Right to Amend: If you feel that the PHI we have is incorrect or incomplete, you may ask us to amend the information.
  • Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures we have made of your PHI.
  • Right to Request Restrictions: You have the right to request a restriction or limitation on the use or disclosure of your PHI for treatment, payment, or health care operations.
  • Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.

 

Changes to This Notice

 

We reserve the right to change this Notice and to make the revised or changed Notice effective for PHI we already have about you as well as any information we receive in the future.

 

Complaints

 

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services.
